Connecting Food & Health project updates

Congratulations to the four Regional Accountable Entities (RAEs) for Accountable Care Collaborative Phase III! They are Colorado Access, Colorado Community Health Alliance, Rocky Mountain Health Plans, and Northeast Health Partners.
Now through July 2025, these RAEs will work with the Colorado Department of Health Care Policy & Financing (HCPF), along with other collaborators, to determine how they’ll fulfill their contracts. The Colorado Blueprint to End Hunger’s Connecting Food & Health project team will continue to remind and equip the RAEs with the importance of centering Member priorities and resourcing food access organizations in their regions.
As HCPF moves towards implementation phases for several of our priorities, monthly meetings for the Connecting Food & Health project will pause until January 2025. However, we’re not taking a break from advocacy efforts!
Please complete this form by Friday, November 8 to help the project team create a foundation for advocacy letters for each RAE. We want to gather input from Medicaid members, community-based organizations, and healthcare clinicians on how they should be fulfilling their contracts when it comes to food and nutrition access for their Members.
