Are you a Medicaid member who wants to be a part of decisions that impact your community? Are you a community-based organization that believes connecting food access and healthcare is important?
Join our final Connecting Food & Health project team meeting on Monday, February 24, from 10 - 11:30 a.m. on Zoom. Project lead Samantha Fields, along with the Blueprint’s Policy Director Greta Allen, will host the Regional Accountable Entities (RAEs) for the Accountable Care Collaborative (ACC) Phase III during this time.
RAEs are responsible for promoting the health and well-being of Medicaid members in their regions. They establish networks of providers, coordinate care, and promote community-based services for member health. Many Medicaid members, providers, and advocates provided feedback on the role of the RAEs in ACC Phase III.
We want to ensure that project members and network partners are connecting with their RAE and are ready to make a difference when the new Collaborative begins in July 2025.
During this final meeting, you’ll have the opportunity to connect with staff from:
RAE 1: Rocky Mountain Health Plans (10:15 - 10:30 a.m.)
RAE 2: Northeast Health Partners (10:30 - 10:45 a.m.)
RAE 3: Colorado Community Health Alliance (10:45 - 11 a.m.)
RAE 4: Colorado Access (11 - 11:15 a.m.)

While Blueprint projects are timebound, we always ensure that work is not abandoned and our partners can join spaces where their impacts continue.
